Position Summary The Little Shell Tribal Health Clinic (LSTHC) seeks a dedicated Community Prevention Specialist to support the health and well-being of Little Shell tribal citizens and other community members through substance use prevention, behavioral health support, community outreach, education, advocacy, and care coordination. This position serves as a trusted liaison between individuals, healthcare providers, social service agencies, schools, community organizations, and prevention coalitions to improve access to services, reduce barriers to care, and strengthen protective factors that promote healthy communities. The Community Prevention Specialist will develop and implement prevention programming, support local and regional prevention initiatives, provide culturally responsive education and outreach, connect individuals to health and social services, and assist with grant reporting and data collection. The position works collaboratively with community partners, coalitions, healthcare providers, and tribal programs to address substance misuse prevention, behavioral health needs, and social drivers of health. Term This position is funded through grant funding. Continued employment is contingent upon funding availability. Essential Duties and Responsibilities Prevention Education and Community Outreach Develop and conduct educational presentations on alcohol, tobacco, other drug prevention, behavioral health, and wellness topics for youth, families, community groups, schools, and organizations. Provide prevention education, health education, and awareness programming utilizing evidence-based and culturally responsive approaches. Conduct community outreach activities that promote healthy behavior and increase awareness of available health and social services. Facilitate media literacy, merchant education, and other prevention-related training courses and educational events. Assist with the development, implementation, and evaluation of regional and local media campaigns focused on prevention and health promotion. Participate in community events, health fairs, trainings, and outreach activities. Community Health Navigation and Advocacy Connect individuals and families with physical health, behavioral health, social services, and community resources. Assist clients with screenings, referrals, follow-up services, and care coordination. Advocate for individuals within healthcare, behavioral health, educational, and social service systems. Promote trust and engagement through culturally informed and trauma-informed interactions. Help individuals identify and overcome barriers to accessing services. Coalition Building and Community Collaboration Participate in and provide technical assistance to local prevention coalitions and community partnerships. Build and maintain collaborative relationships with schools, healthcare providers, tribal programs, social service agencies, community organizations, and other stakeholders. Collaborate with Catalyst for Change and other partners to coordinate prevention initiatives and support community health goals. Support the planning and implementation of environmental prevention strategies, policy initiatives, and community-level interventions. Data Collection, Documentation, and Reporting Maintain accurate and timely records of program activities, client interactions, referrals, training, coalition activities, and outreach events. Collect, record, and report prevention and grant-related data as required by funding agencies, program goals, and reporting guidelines. Document client interactions in accordance with HIPAA, clinic policies, grant requirements, and applicable regulations. Assist with program evaluation and outcome reporting. Professional Responsibilities Attend required meetings, training sessions, conferences, and professional development activities. Maintain confidentiality and professionalism in all aspects of work. Perform related duties as assigned by the Program Manager, Clinic Leadership, or Director. Education and Experience Minimum Qualifications High School Diploma or GED required. Associate degree in Community Health, Social Work, Psychology, Human Services, Health Science, Communication, Public Relations, Behavioral Health, or a related field preferred. At least one (1) year of experience in community health, prevention services, behavioral health, social services, counseling support, community outreach, or a related field. Preferred Qualifications Bachelor's degree in a related field. Experience working with Native American/Alaska Native communities and/or underserved populations. Experience in substance misuse prevention, public health, behavioral health, or community engagement. Experience working with community coalitions, prevention initiatives, or grant-funded programs. Conditions of Employment Must successfully pass a criminal background check. Must pass a DOJ Fingerprint Background check as required to work with minors. Must possess a valid driver's license. Must maintain insurability under Tribal insurance requirements. No alcohol-related driving offenses. Ability to travel locally as required for outreach, meetings, training, and community events.
