The VA Community Outreach Coordinator will be responsible for developing referral relationships and increasing awareness of Carolinas Care Concierge throughout Cumberland County. The ideal candidate will actively engage with organizations and professionals serving veterans, seniors and families while also helping develop our private-pay client base. You will serve as one of the primary faces of Carolinas Care Concierge in the Cumberland County community. Key Responsibilities Develop and maintain relationships with veteran-serving organizations and community partners throughout Cumberland County. Conduct consistent in-person community outreach throughout Fayetteville and surrounding areas. Introduce Carolinas Care Concierge and our home care services to potential referral partners. Build relationships with healthcare professionals, social workers, case managers, discharge planners, physicians, rehabilitation providers, senior-serving organizations and other community partners. Attend veteran events, community events, networking opportunities, health fairs and outreach activities. Identify potential VA and private-pay home care clients. Follow up consistently with prospective clients and referral partners. Educate veterans and families about available non-medical home care services. Help prospective clients navigate the initial referral and intake process. Coordinate incoming referrals with agency leadership and administrative staff. Maintain an organized database of referral sources, leads and follow-up activities. Conduct phone, email and in-person follow-up with referral partners. Distribute marketing materials throughout the community. Identify new organizations, events and partnership opportunities for the agency. Assist with caregiver recruitment and community recruiting efforts when needed. Provide management with weekly reports regarding outreach activities, leads, referrals and new client opportunities.

$47,000.00 - $50,000.00 per year
