Job Description
The Case Manager & Outreach Coordinator plays a dual role in supporting our clients and growing our presence in the community. This position will help eligible seniors and adults with disabilities access and maintain services and programming by helping them navigate state systems, housing networks, and benefit renewals. Additionally, the CMOC will focus on community outreach by building partnerships with local organizations and representing our agency at health fairs and community events and online marketing presence. Core Responsibilities Case Management & Resource Coordination Monitor client Medicaid recertification periods closely. Assist with Medicaid process renewals in advance to prevent any disruptions in their care. Assist client/prospective clients and their families gather financial and administrative paperwork for Medicaid, Social Security updates, and immigration resources. Act as a main point of contact with state administrative agencies to advocate for clients and keep their services running smoothly. Connect clients with essential community support, including housing voucher portals, utility assistance (LIHEAP), SNAP , food pantries, and medical transportation. Coordinate independent living adjustments, such as wheelchair ramps, grab bars, and emergency response buttons. Work with medical and other service partners to facilitate other aspects of care hospital beds, supplies, etc. Community Outreach & Marketing Serve as our primary outreach worker at local host sites and partnership locations Build and maintain relationships with local non-profits, private organizations, and public agencies to share information about our services to include in-home caregiving and non-medical transportation services Grow agency through referral and targeted marketing efforts both Medicaid, Insured, and Private pay clientele. Survey Greater Omaha Area and Satellite locations to identify outreach opportunities. Assist with basic social media, online and print marketing strategies to raise awareness about our services and available programs. Qualifications 19 years of age or older. 1-2 years experience in case management and community engagement. .Must meet all background check and training requirements. Bilingual Spanish and English. Exceptional interpersonal and advocacy skills. Ability to build trust with vulnerable adult populations while professionally representing the agency to external community partners.
