We are seeking an outgoing, bilingual English-Spanish Community Outreach Coordinator to serve as the public-facing representative of Community Care Partners. This role focuses on community outreach, relationship development, event participation, and connecting individuals and families with the services they need. This is an office-based position in Matawan, NJ with frequent travel throughout New Jersey. The schedule may include occasional evenings and weekends to accommodate community events. Job Overview The Community Outreach Coordinator will plan and execute community outreach events, represent Community Care Partners at community and provider events, develop referral relationships, and support growth across CCP's service lines. The ideal candidate is a confident, organized, and highly personable professional with strong public speaking and relationship-building skills. Experience with
NJ DDD, I/DD
services, human services, behavioral health, home care, or another related field is strongly preferred.
Duties / Responsibilities
Plan, organize, and executecommunity outreach events, including community resource fairs, informational sessions, open houses, and family education events, from start to finish. Represent Community Care Partners at community events, provider fairs, transition fairs, school district events, Support Coordination agency meetings, and conferences throughout New Jersey. Build and maintain strong relationships with NJ DDD Support Coordinators and Support Coordination Agencies statewide. Serve as CCP's primary outreach contact for referral partners. Increase referral activity and support census growth across CCP service lines through consistent outreach, timely follow-up, and relationship management. Develop relationships with school districts, hospitals, clinics, community organizations, faith-based organizations, and other referral sources. Provide families with accurate general information about CCP services and direct detailed eligibility and enrollment questions to appropriate internal staff. Promote CCP services, including Community Based Supports, Individual Supports, Employment Supports, Transportation, DDD Transition Services, and the Family Hire Program. Distribute marketing and informational materials and maintain adequate inventory of event supplies. Maintain an outreach calendar and event tracking log, documenting events, contacts, materials distributed, and referrals generated. Coordinate event logistics, including venues, registrations, vendor tables, staffing, setup, and breakdown. Support CCP's social media and community outreach visibility efforts in coordination with leadership. Track and report referral activity, referral source trends, and event outcomes to leadership. Conduct timely follow-up with referral sources, families, and community partners. Represent Community Care Partners professionally and consistently with agency policies, NJ DDD requirements, and HIPAA confidentiality standards. Perform other related duties as assigned. Qualifications Required High school diploma or equivalent. Minimum of two (2) years of experience in community outreach, event planning, marketing, business development, or a related role. Bilingual fluency in English and Spanish, spoken and written . Strong public speaking, communication, interpersonal, and relationship-building skills. Demonstrated ability to plan and execute events from start to finish. Excellent organizational skills and the ability to manage multiple events, relationships, and deadlines independently. Proficiency with Microsoft Office, including Outlook, Word, and Excel. Comfortable using social media platforms. Valid driver's license, reliable personal transportation, and current automobile insurance. Willingness to travel frequently throughout New Jersey. Willingness to work occasional evenings and weekends for community events. Preferred Associate's or Bachelor's degree in Marketing, Communications, Human Services, Business, or a related field. Experience working with individuals with intellectual and developmental disabilities. Familiarity with the NJ DDD service system, Support Coordination, and the provider referral process. Experience in behavioral health, home care, human services, or a related healthcare setting. Previous experience in referral development, community relations, or business development. Travel & Physical Requirements Frequent local and regional travel throughout New Jersey is required. Must be able to transport, set up, and break down event materials and displays weighing up to approximately 25 pounds. Must be able to stand for extended periods during events. Must be comfortable driving and traveling by car for extended periods. Occasional evening and weekend work is required based on community event schedules.
