Starting at $22.00 - $25.00/hour Join Our Team ACR Health is seeking compassionate, community-focused professionals to join our Harm Reduction & Prevention Services team as SEP (Syringe Exchange Program) Program Associates. These positions support participant-centered harm reduction services, overdose prevention initiatives, community outreach, sterile equipment access, referrals to care, and Syringe Exchange Program operations throughout Central and Northern New York. We currently have openings in Syracuse, Syracuse-Oswego, and Watertown.

Syracuse-Oswego SEP Program Associate Focus Areas:

Community outreach and engagement Independent field-based work Relationship-building with rural community partners Ride Along services and post-overdose outreach SEP operations and participant services

Watertown SEP Program Associate Focus Areas:

Regional community outreach SEP operations Participant services Inventory and supply management Community partnership development throughout Jefferson, Lewis, and St. Lawrence Counties Multiple positions available. While all three positions support harm reduction services and SEP operations, each location offers a unique combination of community outreach, participant services, inventory management, and regional service responsibilities. Candidates may apply for one or more locations. What You'll Do Responsibilities may include: Provide participant-centered, stigma-free harm reduction services. Enroll and engage participants in Syringe Exchange Program services. Deliver overdose prevention education and naloxone distribution. Promote safe sharps disposal and access to sterile injection equipment. Connect participants to healthcare, HIV/HCV/STI services, substance use treatment and MOUD, housing resources, food, benefits, behavioral health services, and other community supports. Conduct community outreach, educational presentations, networking activities, and community engagement initiatives. Develop and maintain positive relationships with community partners, healthcare providers, first responders, and local organizations. Participate in outreach events, Ride Along services, and post-overdose engagement activities as assigned. Support daily Syringe Exchange Program operations and service delivery. Maintain inventory of harm reduction, overdose prevention, and SEP supplies. Complete documentation, data collection, reports, and other program requirements. Ensure compliance with confidentiality requirements, agency policies, and applicable regulations. Strong communicators with excellent relationship-building skills. Organized, reliable, and committed to participant-centered service delivery. Respectful, non-judgmental, and culturally responsive in their approach to service provision. Education High School Diploma or GED required. Relevant education in Human Services, Public Health, Social Work, Healthcare, or a related field preferred. Experience Experience in social services, public health, healthcare, harm reduction, outreach, or a related field preferred. Experience working with individuals impacted by substance use, housing instability, justice involvement, mental health concerns, HIV, or other barriers to care preferred. Experience conducting outreach, participant engagement, or community-based service delivery preferred. Experience working with LGBTQIA+, TGNCNB, BIPOC, and other historically underserved communities preferred. Other Requirements Valid New York State driver's license required. Reliable access to an insured vehicle for work-related travel. Ability to travel throughout assigned service areas. Ability to occasionally work evenings and weekends for outreach activities, community events, trainings, and meetings. Skills & Competencies Strong interpersonal and relationship-building skills. Excellent verbal and written communication skills. Effective organizational and documentation skills. Ability to manage multiple responsibilities and priorities. Proficiency with Microsoft Office and electronic recordkeeping systems. Ability to maintain confidentiality and professional boundaries.
