The position equally balances direct client support with community engagement, education, and capacity-building efforts that strengthen community health infrastructure. Working under the direction of the health officer, the CCM serves as a trusted connector between residents, healthcare providers, community organizations, and local government. Through person-centered case management and community outreach, this position helps residents navigate complex systems, address social determinants of health, access available resources, and participate in prevention-focused programs that improve health outcomes and promote healthy aging. The CCM supports the implementation of grant-funded initiatives, including community education, advance care planning, volunteer and facilitator development, and other community health priorities as funding opportunities evolve.

ESSENTIAL DUTIES

Community Case Management (Approximately 50%) The CCM provides individualized support to residents experiencing health-related, social, or economic barriers that impact their well-being.

Responsibilities include:

Conduct comprehensive client assessments utilizing a social determinants of health framework. Develop individualized care plans in partnership with clients, caregivers, and service providers. Connect residents with housing, transportation, food security, healthcare, behavioral health, financial assistance, legal resources, aging services, and other community supports. Coordinate services across healthcare providers, community organizations, local government agencies, and other partners. Conduct home visits, field visits, and community-based meetings as appropriate. Advocate for clients while promoting independence and self-sufficiency. Assist residents in navigating public benefit programs and community resources. Provide ongoing follow-up, care coordination, and case documentation within the department's electronic health record. Maintain client confidentiality and comply with all applicable privacy regulations. Monitor client outcomes and identify barriers requiring systems-level intervention. Participate in interdisciplinary case review and care coordination meetings. Community Engagement, Outreach & Capacity Building (Approximately 50%) The CCM develops and maintains trusted relationships throughout the community to improve awareness of public health services, expand community capacity, and advance prevention-focused initiatives.

Responsibilities include:

Develop partnerships with community organizations, libraries, senior centers, healthcare providers, faith communities, schools, municipalities, businesses, and other stakeholders. Coordinate and facilitate community education programs, workshops, and outreach events. Support implementation of advance care planning initiatives, including education related to Health Care Power of Attorney and Advance Directives. Assist with recruitment, coordination, and support of community-based facilitators, community partners, and volunteers for public health initiatives. Build relationships with culturally diverse communities and reduce barriers to participation in public health programs. Assist with implementation of grant-funded projects, including participant recruitment, event coordination, data collection, evaluation, and reporting. Represent the department at coalition meetings, community events, and collaborative initiatives, including, but not limited to, co-chairing the North Shore Dementia Initiative. Develop and distribute educational materials and resource information. Identify emerging community needs and recommend new programming or partnerships. Support sustainability of community initiatives through relationship development and collaboration. Maintain accurate records of outreach activities, partnerships, participation, and program outcomes. Other duties as assigned.

JOB REQUIREMENTS

Education Requirements :

(Include certificates, licenses, and/or registrations required.). A bachelor's degree from an accredited institution with a major in public health, nursing, human or social sciences, or another closely related field. Master's degree preferred. Individuals that speak languages in addition to English are highly encouraged to apply.

Experience and Other Requirements:

Two years of experience working in case management. At least one year of experience working in a public health setting. Valid Wisconsin driver's license and reliable transportation

Knowledge, Skills, and Abilities:

Extensive knowledge of principles, concepts, and skills of public health. Knowledge of public benefits and resources and crisis intervention. Exceptional communication skills and ability to establish effective working relations with both clients and community partners. Strong clinical skills, including, but not limited to motivational interviewing, active listening, empathic communication, harm reduction, and managing one's own discomfort. Ability to multitask while maintaining a high standard of organization and documentation. Strong cultural competence and a deep appreciation for diversity. Ability to work independently and collaboratively within a multi-disciplinary team.

Pay:

$30.00 - $35.00 per hour
