LOCATION

This is a hybrid position and will require travel throughout Hillsborough County, with varying inperson work locations.

SUMMARY:

The Health Begins Now Navigator provides benefits navigation for a designated group of Hillsborough County residents at clinical locations, community locations, and virtually. The Health Begins Now Program Manager provides day-to-day supervision of this position, with backup supervisory support from the Deputy Director when the Program Manager is unavailable. The Executive Director retains overall organizational oversight of the Health Begins Now program.

ESSENTIAL DUTIES AND RESPONSIBILITIES

(including but not limited to): Provide navigation for social determinants of health needs at clinical settings, community locations, and virtually, as assigned. Maintain expertise in eligibility, enrollment, and program specifications for publicly funded healthcare plans. Conduct public education activities to raise awareness about the Family Healthcare Foundation's services, the Hillsborough County Health Care Plan, Florida KidCare, the Health Insurance Marketplace, and Florida Medicaid. Provide appropriate referrals to all community members when necessary. Provide information in a manner that is culturally and linguistically appropriate to the population being served, including individuals with limited English proficiency. Ensure accessibility and usability of enrollment tools and functions for individuals with disabilities. Assist all community members, whether or not they are a member of the community to be served.

KNOWLEDGE AND EXPERIENCE REQUIRED

Education:

High School Diploma required. Bachelor's preferred.

Experience:

A minimum of five (5) years of experience working in social services and/or public health programming required, case management preferred. Current or prior certification as a Marketplace Navigator or Certified Application Counselor, preferred. Bilingual in Spanish is a plus. Ability to successfully pass a Level II background screening, required. Physical residence in Hillsborough County, strongly preferred.

REMOTE WORK REQUIREMENTS

A private, secure workspace for handling confidential client information while working remotely. Reliable highspeed internet sufficient for videobased client sessions. Use of only FHFapproved/issued devices and platforms for virtual client interactions; not personal accounts or unapproved video/chat apps. Baseline comfort with virtual meeting platforms (Zoom/Teams) and basic troubleshooting.

SKILLS REQUIRED

Practical knowledge and understanding of medical case management and/or social services and social determinants of health. Responsive and effective communicator, both verbally and in writing, with staff, volunteers, and FHF community members. Ability to work independently and exercise sound, independent judgment with minimal direct supervision. Strong organizational, planning, and timemanagement skills, with the ability to manage competing priorities and meet deadlines. Exceptional customer service skills. Ability to make sound decisions that support FHF's mission and vision. Demonstrated proficiency with Microsoft Word, Excel, and Google Drive.

PHYSICAL DEMANDS

Light to moderate physical activity performing activities to include: Sitting, standing, and/or walking for extended periods. Working in a welllighted, heated, and/or airconditioned indoor office setting with adequate ventilation. Working in an outdoor environment as needed for community engagement activities. Using manual dexterity sufficient to handle/reach items. Using close/distant/peripheral and depth perception. Regular travel using a personal vehicle is required. Mileage for work travel is reimbursable.
