Job Description
As a Healthcare Outreach Specialist, your primary focus will be driving referral growth by building relationships with physicians, hospitals, rehabilitation centers, assisted living facilities, community organizations, and other referral sources. You will identify new business opportunities, strengthen existing partnerships, and educate healthcare professionals and community leaders on the services CDAA provides. This position is ideal for a motivated professional who enjoys networking, building long-term relationships, and achieving measurable growth goals. Success in this role is measured by your ability to consistently increase referral volume, establish new community partnerships, and contribute to the organization's overall growth. Duties and Responsibilities Develop and maintain relationships with physicians, hospitals, rehabilitation facilities, skilled nursing facilities, adult day health centers, assisted living communities, behavioral health providers, and community organizations. Identify and cultivate new referral sources that generate consistent patient referrals. Conduct daily in-person visits to healthcare providers and community partners within assigned territories. Represent CDAA at networking events, health fairs, provider meetings, and community outreach events. Educate referral partners on CDAA's programs and available healthcare services. Generate new referral opportunities through proactive outreach and relationship-building. Meet and exceed monthly referral and business development goals. Maintain regular communication with existing referral partners to strengthen long-term relationships. Collaborate with the Intake and Operations teams to ensure timely processing of referrals. Research market trends and identify opportunities to expand into new communities and service areas. Develop strategic outreach plans to increase community awareness and organizational growth. Track outreach activities, referrals, and partnership development using company systems. Prepare weekly reports detailing outreach activities, referral performance, and business development efforts. Assist leadership in identifying new partnership opportunities and strategic growth initiatives. Maintain compliance with company policies and applicable healthcare regulations. Required Experience/Skills Bachelor's degree in Business Administration, Healthcare Administration, Public Health, Marketing, Communications, or a related field preferred. Minimum of 2 years of experience in healthcare outreach, provider relations, healthcare sales, business development, or community engagement. Demonstrated ability to develop referral networks and build professional relationships. Strong networking, presentation, and interpersonal communication skills. Proven ability to meet performance goals and drive measurable business growth. Excellent organizational and time management skills. Self-motivated with the ability to work independently and manage multiple priorities. Comfortable traveling throughout assigned territories for community outreach and provider visits. Proficient in Microsoft Office and CRM or referral tracking systems. Valid driver's license and reliable transportation required. Bilingual candidates (Spanish, Mandarin, Bengali, Creole, or other languages) are strongly encouraged to apply. Performance Expectations Success in this role will be measured through key performance indicators (KPIs), including: Weekly Complete 20-30 in-person provider visits. Establish 3-5 new referral partner meetings. Attend networking events or community outreach activities as scheduled. Submit weekly outreach activity reports. Monthly Generate new qualified referrals that meet departmental goals. Establish 5-10 new referral partnerships. Maintain regular communication with existing referral partners. Achieve monthly outreach and business development objectives established by leadership. Quarterly Increase referral volume within assigned territory. Expand strategic partnerships with healthcare providers and community organizations. Identify new growth opportunities and underserved markets. Demonstrate measurable contribution to organizational growth. Performance metrics may be adjusted based on territory, service lines, and organizational growth initiatives.
