Job Description
Overview We are seeking a dedicated and compassionate Outreach Coordinator/Entry Level Case Manager to serve in Baltimore, focusing on supporting individuals with severe and persistent mental illness. The ideal candidate will possess some experience in the behavioral health field, with a passion for empowering individuals through person-centered approaches. This role offers an opportunity to make a meaningful difference by connecting clients with vital resources, providing crisis management, and fostering sustainable independence. The Outreach Coordinator provide transportation services for clients attending our program. Must be familiar with the South Baltimore area and comfortable driving a 15 passenger van (training provided). Must have active CPR certification. Responsibilities Conduct outreach activities to engage clients, and build trust and rapport within the community. Assess client needs utilizing case management principles to develop personalized service plans. Maintain detailed documentation of client interactions, service plans, and progress using data entry systems and Microsoft Office tools. Manage crisis situations effectively by implementing appropriate intervention strategies while ensuring safety for clients and staff. Ensure compliance with organizational policies and federal/state regulations Proficiency in Microsoft Office applications for documentation, reporting, and communication purposes. Experience working with culturally diverse communities, demonstrating cultural competency and sensitivity. Driving clients to and from program site.
