Care Coordinator / Client Assessment & Marketing Specialist About Pure Comfort Homecare LLC Pure Comfort Homecare LLC is a professional non-medical home care agency dedicated to providing compassionate, dependable, and personalized care to individuals and families in the comfort of their homes. We are committed to helping clients maintain dignity, independence, safety, and quality of life while providing families with dependable support. We are seeking a motivated, compassionate, and professional Care Coordinator to join our growing team. This position combines client assessments and care coordination with community outreach, referral development, and marketing . Position Summary The Care Coordinator will serve as an important connection between prospective clients, existing clients, caregivers, healthcare professionals, families, and community referral partners. The successful candidate will conduct client assessments, gather information regarding clients' needs, assist with developing appropriate care plans, coordinate services, maintain communication with clients and families, and help grow the agency through professional marketing and community outreach. This position is ideal for someone who is comfortable working independently, communicating with families and healthcare professionals, conducting in-home visits, networking within the community, and representing the agency professionally. Essential ResponsibilitiesClient Assessments & Care Coordination Conduct initial and follow-up client assessments in the client's home or other appropriate setting. Meet with prospective clients and families to identify care needs, preferences, schedules, safety concerns, and service expectations. Gather and document client information accurately and professionally. Assist with developing individualized service plans based on assessed needs and authorized services. Coordinate caregiver assignments and communicate client needs to appropriate team members. Maintain regular communication with clients, families, caregivers, and referral partners. Monitor client satisfaction and identify changes in care needs. Conduct follow-up calls and visits as needed. Communicate changes in client condition, service needs, scheduling concerns, or safety issues to appropriate agency leadership. Assist with client intake, service initiation, transitions, and discharge processes. Maintain accurate and timely documentation in accordance with company policies and applicable requirements. Protect client confidentiality and comply with HIPAA and agency privacy policies. Marketing & Community Outreach Develop and maintain professional relationships with hospitals, rehabilitation facilities, assisted living communities, memory care communities, physicians' offices, hospice organizations, social workers, case managers, senior service organizations, VA-related resources, and other referral sources. Conduct community outreach and represent Pure Comfort Homecare at networking events, health fairs, senior events, professional meetings, and community activities. Make scheduled marketing visits to potential referral partners. Deliver brochures, business cards, informational materials, and other approved marketing materials. Educate referral partners and families about the agency's non-medical home care services. Follow up with referral sources and prospective clients. Track referrals, leads, outreach activities, and marketing contacts. Assist with developing new referral relationships and increasing awareness of Pure Comfort Homecare's services. Support social media, community campaigns, promotional activities, and other approved marketing initiatives. Identify opportunities to expand the agency's presence within the communities we serve. Maintain a professional image and represent the company's values at all times. Client Relations Provide exceptional customer service to clients and families. Respond to questions and concerns professionally and promptly. Build trusting relationships with clients and their families. Help families understand available home care services and payment options. Assist families with navigating the intake and service-start process. Promote client satisfaction and retention through consistent communication and follow-up. Qualifications High school diploma or equivalent required. Associate's or bachelor's degree in healthcare, human services, business, marketing, social services, or a related field preferred. Previous experience in home care, healthcare, senior services, case management, care coordination, sales, marketing, community outreach, or a related field preferred. Experience conducting client assessments or intake interviews preferred. Strong interpersonal and communication skills. Professional telephone and written communication skills. Ability to build relationships with healthcare and community professionals. Strong organizational and time-management skills. Ability to work independently and manage multiple priorities. Comfortable traveling throughout the agency's service area for assessments, meetings, and outreach activities. Valid driver's license, reliable transportation, and appropriate auto insurance required. Ability to maintain confidential client information. Proficiency with computers, email, Microsoft Office/Google Workspace, electronic documentation systems, and social media platforms preferred. Preferred Experience Candidates with experience as a Care Coordinator, Case Manager, Community Outreach Coordinator, Home Care Liaison, Healthcare Marketing Representative, Admissions Coordinator, Patient Care Coordinator, CNA/PCA, Social Worker, Medical Assistant, or Healthcare Sales Representative are encouraged to apply. Participate consistently in community outreach and marketing activities. Follow up with prospective clients and referral sources. Represent Pure Comfort Homecare professionally and ethically. Support the agency's growth while maintaining a client-centered approach. Physical & Work Requirements Ability to travel to client homes and community locations. Ability to sit, stand, walk, and communicate for extended periods. Ability to use a computer and telephone regularly. Ability to occasionally work evenings or weekends for community events, client needs, or agency activities. Must be comfortable working in a variety of client home environments.
