This position will act as a key connection between our facility, local hospitals, healthcare providers, patients, and families by helping coordinate referrals, evaluate potential admissions, and support smooth transitions of care. This position requires someone who is comfortable building relationships within the Pensacola healthcare community and following referrals from initial contact through admission.
