Clinical Liaison - RN | Pensacola, FL We are currently accepting applications for a Registered Nurse (RN) Clinical Liaison to serve the Pensacola, Florida area . This position will act as a key connection between our facility, local hospitals, healthcare providers, patients, and families by helping coordinate referrals, evaluate potential admissions, and support smooth transitions of care. The ideal candidate will be professional, dependable, organized, and highly motivated, with strong clinical judgment and excellent communication skills. This position requires someone who is comfortable building relationships within the Pensacola healthcare community and following referrals from initial contact through admission. We are a subsidiary of Crowne Health Care , an Alabama-based system of family-owned nursing facilities. Benefits Competitive Pay - Pay Scale Based on Experience 401(k) and 401(k) Matching Health Insurance Dental Insurance Vision Insurance Life Insurance Disability Insurance Flexible Spending Account Paid Time Off Holiday Pay with No Waiting Period Qualifications Current, active Registered Nurse (RN) license Associate or bachelor's degree in nursing Clinical, admissions, case management, discharge planning, or long-term care experience preferred Strong clinical assessment and decision-making skills Excellent communication, organization, and follow-up skills Ability to build and maintain professional relationships with hospital staff, case managers, physicians, patients, and families Knowledge of skilled nursing and rehabilitation services preferred Ability to review medical records and clinical information to help determine appropriate placement Essential Job Functions Develop and maintain relationships with hospitals, case managers, discharge planners, physicians, and other referral sources throughout the Pensacola area Respond promptly to referrals and maintain consistent follow-up throughout the referral and admission process Conduct clinical pre-screenings of prospective residents to determine appropriateness for admission Review medical records, diagnoses, medications, treatments, therapy needs, and other pertinent clinical information Communicate with facility leadership and clinical teams regarding prospective admissions Review payer and insurance information and help ensure admission requirements are met Coordinate patient discharges, transfers, and admissions Meet with prospective residents, family members, and caregivers to explain available skilled nursing and rehabilitation services Promote the facility and its services to appropriate referral sources and prospective residents Identify opportunities to increase qualified referrals and admissions Represent Crowne Health Care professionally throughout the Pensacola healthcare community Maintain confidentiality and comply with HIPAA, federal, state, and facility requirements We are looking for an RN who is a motivated self-starter, enjoys building relationships, understands the importance of timely follow-up, and is committed to helping patients and families navigate the transition into skilled nursing and rehabilitation care. #IND123M
