Experience Minimum of two years of experience in your licensed clinical discipline. Rehabilitation, LTACH, acute care, or healthcare marketing/business development experience preferred. Strong communication, relationship-building, organization, and presentation skills. Ability to work collaboratively with clinical and business development teams. What You'll Do Working under the direction of the Director of Business Development , the Clinical Navigator serves as a clinical and relationship-building resource throughout the referral and admission process. In this role, you will: Build and maintain strong referral relationships within an assigned territory. Serve as a clinical resource and representative of PAM Health to patients, families, physicians, case managers, discharge planners, nurses, and other referral partners. Complete clinical assessments of prospective patients to determine appropriateness for admission. Cultivate new referral opportunities and maintain existing referral relationships. Analyze referral and admission trends to identify opportunities for growth. Collaborate closely with the Business Development team, including the Admissions Manager and admissions staff, to facilitate referral conversion and admissions. Partner with nursing, therapy, physicians, and other internal teams to support a seamless transition of care. Educate patients, families, and referral sources about PAM Health's programs, services, and clinical capabilities. Develop and maintain strong relationships with healthcare stakeholders, including physicians, ICU and floor nurses, case managers, discharge planners, payer representatives, patients, and families. Provide feedback and recommendations to support program development, quality improvement, and exceptional customer service. Monitor patient and referral source satisfaction and identify opportunities to improve the referral and admission experience. Represent PAM Health professionally throughout the community and assigned territory. Education & Licensure Degree in a health-related field from an accredited college or university preferred. Current professional license in one of the following fields required: Registered Nursing (RN) Respiratory Therapy (RT) Physical Therapy (PT) Occupational Therapy (OT) Speech-Language Pathology (SLP)
