Job Description
Admissions & Social Services Support MWThFSat/Sun Full Time Job Description Maintain accurate admission records and resident information. Communicate with residents, families, hospitals, referral sources, and facility staff regarding admissions. Assist with managing referrals, waiting lists, and prospective admissions. Support census tracking and admission reporting. Provide administrative support to the Admissions and Social Services departments. Assist with resident assessments, care conferences, and required documentation. Support residents and families by providing information, resources, and assistance throughout their stay. Assist with discharge planning and coordination of community resources. Help address resident and family concerns and report issues to the appropriate department. Build and maintain positive relationships with hospitals, physicians, insurance providers, and community partners. Assist with marketing, community outreach, and referral development activities as assigned. Maintain resident confidentiality and comply with HIPAA, facility policies, and state and federal regulations. Attend meetings, trainings, and in-service education as required. Perform other duties as assigned by the Director of Admissions, Director of Social Services, or Administrator. Qualifications High school diploma or equivalent required; Associate's degree in Healthcare Administration, Human Services, Social Work, or related field preferred. Previous experience in healthcare, admissions, customer service, social services, or long-term care preferred. Strong organizational, communication, and multitasking skills. Proficiency with Microsoft Office and electronic medical record systems preferred. Ability to interact professionally and compassionately with residents, families, hospitals, and community partners. Ability to maintain confidentiality and handle sensitive information appropriately. Ability to work independently while collaborating effectively with an interdisciplinary team.
