Friday•MondayFriday 12-6, Saturday 9-3 Sunday 9-3 Monday 12-6The Admissions Counselor is responsible for the processing and admitting of individuals with services at Masonicare Health Center in accordance with financial policies and procedures.
Essential Duties and Responsibilities:
Responds to admission inquires via hospital, referral source, telephone or walk in and provides pertinent information.. Maintains and fosters public relations with customers, internal and external.Assesses individual/ financial status, evaluates care needs evidenced by medical/ functional information presented, and assigns level of care so that the applicant is placed on the wait list for the appropriate service area.
Provides supportive care and guidance to applicants and their families for continuing care needs for community or institutional servicesEnsures
PASSR/ ASCEND
process is completed by appropriate staff for new residents for admission and ensures that the financial eligibility has been met.Coordinates pre-admission/ admission conference with representative of the care center team, i.e., director, care coordinator, and social worker to ensure transfer of medical, social, and financial/ insurance information to the care teamScreens referrals of patients from hospital and community for admission to rehab/ sub acute units and coordinates acceptance of their admission with care center team, i.e., care coordinator, social worker, and clinical coordinator.
Maintains waitlist for applications to long term care units and works with Social Worker for processing to Finance Department.
Signs-in all long term care applicants on admission and copies and distributes paperwork accordinglyPerforms other duties as assigned.
