Nesconset Salary:
Up to $100,000 per year Join Our Team as a Marketer / Hospital Liaison! The Hamlet Rehabilitation and Healthcare Center at Nesconset is seeking an experienced, energetic, and relationship-driven Marketer / Hospital Liaison to support census growth and strengthen referral partnerships throughout the healthcare community. The ideal candidate will have experience in skilled nursing, rehabilitation, long-term care, or healthcare marketing and possess a strong understanding of the post-acute care continuum. Position Summary The Marketer / Hospital Liaison serves as the primary point of contact for hospitals, case managers, physicians, discharge planners, and community partners to promote the facility's clinical programs and services. This role is responsible for developing referral relationships, increasing admissions, and ensuring a seamless transition for patients entering our center. Responsibilities Develop and maintain relationships with hospitals, physicians, discharge planners, case managers, and community organizations. Educate referral sources about the facility's services, specialty programs, and admission process. Conduct routine visits to hospitals, physician offices, and community partners within the assigned territory. Identify opportunities to increase referrals and support census development initiatives. Coordinate with the Admissions and Clinical teams to facilitate timely and appropriate patient admissions. Track referral activity and maintain accurate records of outreach efforts. Attend networking events, healthcare meetings, and community functions to promote the facility. Collaborate with leadership to develop and execute strategic marketing plans. Maintain a thorough understanding of payer sources, managed care plans, and post-acute care services. Ensure exceptional customer service throughout the referral and admission process. Qualifications Minimum of 2 years of healthcare marketing, business development, admissions, or hospital liaison experience. Experience in a Skilled Nursing Facility, Rehabilitation Center, Long-Term Care, Home Health, Hospice, or Hospital setting preferred. Established relationships with local hospitals and healthcare providers are a plus. Strong knowledge of the post-acute care continuum and discharge planning process. Excellent communication, presentation, and interpersonal skills. Self-motivated with strong organizational and time management abilities. Valid driver's license and reliable transportation required. Ability to travel throughout the local market.
