You will:
Coordinate Clinical Care:
Work closely with interdisciplinary teams and external providers to ensure seamless updates on clinical information.
Drive Treatment Planning:
Lead the admission process by completing thorough psychosocial assessments and helping build a roadmap for recovery. Connect the
Community:
Provide vital resource information and "warm-referrals" to community providers to ensure ongoing engagement.
Manage Admissions:
Handle the intake process with excellence, including verification of benefits and necessary clinical authorizations.
What We're Looking For:
Bachelor's degree (LSW or RN preferred), or Master's degree in social work or counseling with licensure. Must be 21 yrs or older.
License:
BSW, LSW, LISW, LPC, LPCC, MFT
Mental Health Therapist Social Services Licensed Professional Counselor
