Job Description
Key Responsibilities:
Relationship Building:
Establish and maintain positive relationships with hospital case managers, social workers, discharge planners, and other healthcare professionals. Actively engage with the local community and healthcare providers to promote the facility's services and capabilities. Represent the facility at community events, health fairs, and professional networking meetings.Referral Management:
Identify and develop new referral sources within hospitals and the local community. Relay information from the hospital to the Admissions Director for clinical and financial review. Collaborate with the Admissions Director to ensure a smooth and efficient admission process for patients and their families.Admission Conversion:
Convert hospital referrals to admissions by facilitating communication and coordination between hospital staff and the facility. Ensure that all necessary information is provided to the Admissions Director for decision-making. Follow up on referral status to maximize conversion rates and ensure satisfaction.Marketing and Outreach:
Implement effective marketing strategies to increase awareness of the facility's services. Conduct presentations and informational sessions for hospital staff and community organizations. Provide educational materials and resources to referral sources and potential patients.Communication and Coordination:
Serve as the primary point of contact for hospital staff and referral sources. Facilitate communication between hospital staff, patients, families, and the care team to ensure continuity of care. Monitor and follow up on referral status to ensure high conversion rates and satisfaction.Qualifications:
Previous experience in a hospital liaison, admissions, marketing, or sales role within a healthcare setting. Understanding of skilled nursing and rehabilitation services. Excellent interpersonal and communication skills, with the ability to build rapport with healthcare professionals and community partners. Proven ability to manage multiple priorities and work independently in a fast-paced environment. Proficiency in Microsoft Office Suite and CRM software.Pay:
