- Hospice Homecare Job Summary Maintains a professional image and exhibits excellent customer relations to patients, visitors, physicians, and co-workers in accordance with our Service Excellence Standards and Core Values.
Serves as the admissions liaison for McLeod Hospice. Receive hospice referrals from hospitals, physicians, facilities, and community sources. Meet with patients and families (in person or virtually) to explain hospice philosophy, services, eligibility basics, and the plan of care in a compassionate, understandable manner. Provide emotional support and answer questions to help families make informed decisions regarding hospice care. Facilitate the hospice election and enrollment process when patients/families choose hospice. Obtain required consents, signatures, and documentation in accordance with agency policy. Accurately complete and submit admission-related paperwork and demographic information. Communicate enrollment details promptly to the interdisciplinary clinical team to enable timely initiation of start-of-care visits. Coordinate with intake, scheduling, and clinical leadership to ensure smooth transitions into hospice services. Identify and escalate clinical concerns appropriately to licensed staff. Support community liaison's marketing and referral development efforts by serving as the hospice admissions liaison to acute care facilities, including hospitals, emergency departments, and inpatient units
- following referral receipt, focusing on patient/family education, enrollment and ongoing hospital relationship support.
Build and maintain professional relationships with case managers, social workers, discharge planners, and physicians. Supports referral development efforts by educating referral partners on hospice services, eligibility criteria, and admissions processes, consistent with non-marketing admissions responsibilities. Complies with applicable federal, state, and organizational hospice regulations, policies, and guidelines. Participate in audits, quality initiatives, and performance improvement efforts related to admissions and referral processes.
Work Schedule:
80 hours bi-weekly. Day Shift. Rotating weekend work required. Qualifications /
Training:
Experience in healthcare admissions, patient access, care coordination, social services, or healthcare sales/liaison work. Strong communication and interpersonal skills, with the ability to discuss end-of-life care sensitively and respectfully. Excellent organizational skills and attention to detail. Ability to work independently and collaboratively in a fast-paced healthcare environment. Valid driver's license and reliable transportation (if field-based). Current South Carolina Driver's License and Automobile Insurance as required by law .
Licenses/Certifications/Registrations/Education:
Bachelor's degree in related field. Associates degree with experience in lieu of bachelor's will be considered.
