A Family, School, or General Social Worker provides specialized assistance to vulnerable families and children. Intervenes in crisis situations; evaluates individual and family needs, helps clients get access to health care, mental health care or social services such as housing assistance. Advocates for clients and provides follow up visits. May work with teachers, parents and school administrators to help students improve academically and deal with social issues such as bullying.
Overview Facilitates resource management activities for inpatients based on identified patients. Coordinates patient's transition from hospital to home through collaboration with physicians, nursing staff and other healthcare professionals to enhance continuity of patient care, promote cost effective utilization of resources and enhance patient, nurse and physician satisfaction. Provides psychosocial assessment, resources and intervention to hospitalized patients.
Qualifications Education:
Require BS degree in Social Work from an accredited college or university and current Alabama licensure.
Experience:
Two years hospital experience preferred. Comparable combination of education and experience will be considered. Previous discharge planning or hospital based social service experience very beneficial.

Social Workers are expected to demonstrate excellent communication skills, to be capable of working with a diverse population and to behave ethically and professionally. Social Workers interact with professionals from many different social service and governmental agencies, so must be able to collaborate well with others. Because social work frequently involves crises, the social worker must demonstrate emotional resilience and stability.
