Skilled Nursing Facility Department:

Full Time Location:
Fair Haven Description A Licensed Social Worker (LSW) in a skilled nursing facility (SNF) provides comprehensive psychosocial assessments, care planning, and therapeutic interventions to support residents' mental and emotional well-being. They facilitate admissions/discharges, advocate for resident rights, and mediate between families and the interdisciplinary care team.
Key Responsibilities Key Responsibilities Psychosocial Assessments & Care Plans :
Conduct comprehensive assessments upon admission and develop individualized, 90-day, or annual care plans.
Discharge Planning :
Coordinate with insurance managers, families, and community resources to manage safe discharges.
Resident Advocacy & Rights :
Ensure compliance with federal/state regulations, promote resident dignity, and protect rights.
Family/Resident Support :
Offer crisis intervention, counseling, and education to help residents adjust to long-term care.
Interdisciplinary Collaboration :
Participate in daily rounds, care conferences, and team meetings to review clinical, emotional, and social needs.
Skills, Knowledge and Expertise Required Qualifications Education:
Bachelor's or Master's degree in Social Work (BSW/MSW) from an accredited program.
Licensure:
Valid state licensure (LSW, LCSW, or equivalent).
Experience:
Previous experience in a nursing home, hospital, or geriatric care is preferred.
Skills:
Strong communication, documentation, and crisis management skills.
