Tuscaloosa, AL Job Details Full-time 1 day ago Qualifications Driver's License Bachelor's degree Full Job Description We are seeking a compassionate and dedicated Social Work/Integrated Care professional to support patients with their psychosocial, behavioral health, and social service needs. This position provides brief counseling and intervention, coordinates referrals, and connects patients with medical and community resources. Key Responsibilities Assess patients' psychosocial needs and provide brief intervention and counseling. Use Motivational Interviewing, CBT, and crisis intervention techniques. Collaborate with physicians, nurses, and other healthcare professionals as part of an integrated care team. Complete assessments, treatment plans, progress notes, and required documentation. Coordinate referrals and connect patients with community and supportive services. Provide HIV and HCV counseling, testing, and referrals. Assist with Ryan White Program eligibility, CAREWare, ServicePoint, and related data entry. Maintain strict confidentiality and comply with HIPAA and agency policies. Participate in professional development and continuing education. Perform other duties as assigned. Master's degree in Social Work preferred; Bachelor's degree required Social Work license preferred 3-5 years of experience in the field or related work, especially in a medical setting Familiarity with a variety of the field's concepts, practices, and procedures Effective written, verbal, and presentation communication skills Ability to organize and coordinate a variety of tasks and responsibilities Valid Alabama Driver's License required Prior academic or practical training in motivational interviewing, cognitive behavioral therapy, or Brief Therapy/Crisis Intervention requiredBenefits
