8:00am - 5:00pm
Summary:
The PACE Social Worker provides direct case management services to participants. Responsibilities include participant screening and assessments, social service support, counseling, and referrals.
Other information:
Master's degree required LMSW or LCSW required Two years experience in healthcare preferred Experience working with the frail and elderly population preferred Excellent customer service skills Excellent interpersonal communication skills This job will be authorized 80.00 hours bi-weekly.
