A Family, School, or General Social Worker provides specialized assistance to vulnerable families and children. Intervenes in crisis situations; evaluates individual and family needs, helps clients get access to health care, mental health care or social services such as housing assistance. Advocates for clients and provides follow up visits. May work with teachers, parents and school administrators to help students improve academically and deal with social issues such as bullying.
We are seeking a compassionate, organized, and motivated Social Director to join our long-term care team. The Social Director plays a vital role in enhancing the quality of life for our residents by providing emotional support, assisting with psychosocial needs, coordinating admissions and discharges, and serving as an advocate for residents and their families. Responsibilities Complete psychosocial assessments and care plans for residents. Assist residents and families with adjustment to long-term care placement. Coordinate admissions, discharges, and transfers. Facilitate care plan meetings and participate as an interdisciplinary team member. Provide counseling, crisis intervention, and emotional support as needed. Connect residents and families with community resources and support services. Maintain accurate documentation in accordance with state and federal regulations. Assist with resident rights, grievance resolution, and discharge planning. Build positive relationships with residents, families, physicians, and staff. Ensure compliance with CMS, state, and facility policies. Qualifications Bachelor's degree in Social Work, Human Services, Psychology, or a related field preferred. Experience in long-term care, skilled nursing, healthcare, or senior services preferred. Excellent communication and interpersonal skills. Strong organizational and documentation abilities. Ability to work independently while collaborating with an interdisciplinary team. Knowledge of Medicare, Medicaid, and long-term care regulations is a plus.
