Job Description
NEEDED:
We have a PRN position available. The branch is led by a local RN, and we have an amazing team. We are dedicated to providing compassionate hospice care to those in need. Hospice experience is strongly preferred. Must be willing to travel to wherever the patient calls home! This is a PRN position for Globe/Miami and surrounding areas. PRN teammates are paid per visit + Mileage is reimbursed. Bereavement calls and training time are paid by the hour. - Conduct assessments and evaluations of individuals to determine their needs and develop appropriate treatment plans including education and planning for end of life wishes.
- Collaborate with other professionals, such as Nurses, Physicians, Chaplains and educators, to develop comprehensive treatment plans
- Provide emotional support to individuals and families during end of life situations.
- Assist individuals in accessing community resources and support services
