About us THEMA Health Services (Hospice) is looking for an extraordinary social worker (MSW) with a hospice heart who wants to work for a company that cares. Due to growth we are seeking quality people join us in the delivery of exceptional end of life care.
NEEDED:
We have a PRN position available. The branch is led by a local RN, and we have an amazing team. We are dedicated to providing compassionate hospice care to those in need. Hospice experience is strongly preferred. Must be willing to travel to wherever the patient calls home! This is a PRN position for Globe/Miami and surrounding areas. PRN teammates are paid per visit + Mileage is reimbursed. Bereavement calls and training time are paid by the hour. Duties:
  • Conduct assessments and evaluations of individuals to determine their needs and develop appropriate treatment plans including education and planning for end of life wishes.
  • Collaborate with other professionals, such as Nurses, Physicians, Chaplains and educators, to develop comprehensive treatment plans
  • Provide emotional support to individuals and families during end of life situations.
  • Assist individuals in accessing community resources and support services

Qualifications Master of Social Work (MSW) degree required Strong skills in patient assessment and care Proficient in medical documentation and terminology Experience in hospice care and behavioral therapy Knowledge of HIPAA regulations and medical records management Familiarity with crisis intervention and psychotherapy techniques Ability to provide clinical counseling and support to patients and families
