Job Description Manage a caseload of 6-8 youth with weekly contact and monthly face-to-face visits Coordinate care for youth in foster homes and independent living, including school, medical, behavioral health, and community services Support life skills development such as budgeting and preparation for independent living Prepare youth for court, coordinate with attorneys, and provide transportation/support as needed Develop and update Individual Service Plans and Independent Living Plans Lead quarterly Child and Family Team ( CFT ) meetings with the full support team Complete detailed case documentation, reports, and required incident records Provide on-call emergency coverage and occasionally accompany youth on domestic travel We are a company committed to creating diverse and inclusive environments where people can bring their full, authentic selves to work every day.
