• LMSW Social Worker

POSITION DUTIES

• Provides comprehensive inpatient and outpatient care coordination for high-risk patients by conducting assessments, developing care plans, connecting patients with community, behavioral health, and healthcare resources, and advocating for their needs.

Working as part of an interdisciplinary team, the Social Worker evaluates medical necessity, supports discharge planning, coordinates services across the continuum of care, monitors patient progress, and promotes safe, efficient, and cost-effective care for patients of all ages, including those with mental health and complex psychosocial needs.

MINIMUM REQUIRED QUALIFICATIONS

• Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW).

A valid, current, full and unrestricted Professional License to practice LMSW or LCSW in any state of the United States of America, The Commonwealth of Puerto Rico, or a territory of the United States. Master's degree in social work AHA BLS 3+ years' experience in the clinical care setting Experience and knowledge of discharge planning, case management, or utilization review processes Demonstrate knowledge of electronic health record systems
