PACE PACE
(Program of All-Inclusive Care for the Elderly) is a comprehensive care model for individuals age 55+ who are nursing home-eligible but wish to remain safely living in their homes. PACE provides coordinated medical and social services through an Interdisciplinary Team (IDT) , with a focus on prevention, care coordination, chronic condition management, and overall quality of life.

  • 56/hour Position Summary The Social Worker (MSW) provides comprehensive psychosocial support to PACE participants as part of the Interdisciplinary Team (IDT).
This role is responsible for conducting psychosocial assessments, supporting care planning, providing counseling and education, coordinating resources, and advocating for participants and their families throughout their care. Key Responsibilities Conduct scheduled and unscheduled psychosocial assessments and evaluations. Provide individual and family counseling, education, and emotional support. Identify and address behavioral health and psychosocial needs. Provide case management, advocacy, and discharge planning support. Facilitate family conferences and therapeutic support groups. Coordinate advance care planning in collaboration with the PCP and IDT. Connect participants and families with community resources, including housing, financial assistance, legal services, and mental health resources. Participate in home visits as needed. Maintain accurate, timely, and compliant documentation. Participate in IDT meetings, care planning, and ongoing participant evaluations. Advocate for participants while promoting their independence, safety, and overall quality of life. Maintain compliance with Social Work Code of Ethics, HIPAA, and applicable regulatory requirements. Skills & Qualifications Strong written and verbal communication skills. Excellent organization, time management, and prioritization skills. Strong interpersonal, customer service, and facilitation skills. Ability to work collaboratively as part of an interdisciplinary team. Self-motivated, adaptable, and detail-oriented. Proficiency with Microsoft Office, including Word and Excel. Knowledge of DHCS, CMS, and HIPAA regulations. Education & Experience Master's Degree in Social Work (MSW) from an accredited university
  • required. Minimum 2 years of experience in case management, social advocacy, and/or mental health services, preferably working with a geriatric population. Relevant fieldwork/internship experience may substitute for up to 1 year of required experience. Bilingual English/Spanish
  • required. CPR/First Aid certification
