Job Description

MSW/ Social Worker Responsibilities:

Intake screen (Check eligibility, fill out intake form, provide tour, etc.) Conduct initial assessments, bi-annual reassessments. Conduct initial home evaluations and home visits as needed. Individual and group counseling to elderly participants Connect participants with supporting resources, such as IHSS and Access Paratransit. Provide case management. Complete quarterly assessment, progress notes, flow sheets in a timely manner. Planning and coordination of discharges and manage discharge folders. Bilingual (spoken and/or written) in Chinese (Mandarin and/or Cantonese) Rotating Saturdays. H1b sponsorship available if qualify.
