Pay:

$32.00-$43.00/hour

Social Worker Opportunity:

Full-time, permanent position Monday-Friday, working 8:00AM-5:00PM The Social Worker will assess patients' psychosocial needs and provide counseling, emotional support, and individualized care planning Connect patients and families with community resources while advocating for their social, financial, and support needs The Social Worker will collaborate with the interdisciplinary care team to coordinate care, maintain documentation, and support quality patient outcomes Social Worker Requirements Master's Degree in Social Work Required LCSW's are preferred but LMSW's, CSWA's, and ACSW's are welcome to apply
