You will help participants overcome barriers to accessing wellness, medical, and rehabilitative care, and you will work closely with the Program Director on the daily operations of the center. Key Responsibilities Conduct participant assessments, develop individualized care plans, and advocate on behalf of each participant Research community resources and coordinate referrals Maintain accurate and timely case documentation Participate in and help lead the coordination of social and health services Take part in multidisciplinary team meetings and consult with therapists, families, and participants Support the Program Director with the day-to-day management of the ADHC Assist with staff supervision, audit preparation, and reporting Perform other duties as assigned Qualifications Master of Social Work (MSW), or Bachelor of Social Work (BSW) plus two years of healthcare-related experience Ability to work with individuals with physical, cognitive, or mental health limitations Valid California driver's license and proof of auto insurance New graduates are welcome to apply Preferred Bilingual in English and Chinese (Mandarin and/or Cantonese) Previous experience in a
