acute hospital Social Work experience Responsibilities Conduct thorough patient assessments to identify psychosocial needs, barriers to care, and appropriate intervention strategies Provide individual, group, and family counseling sessions utilizing evidence-based practices such as cognitive behavioral therapy, motivational interviewing, and behavioral therapy Coordinate community resources and support services to facilitate discharge planning and ongoing care management
