JOB DESCRIPTION SUMMARY

We are seeking a compassionate and dedicated Hospice Medical Social Worker to provide essential psychosocial support to patients and families facing end-of-life challenges. As an integral member of our interdisciplinary team, you will assess and address the emotional, social, and practical needs of patients, while collaborating to ensure comprehensive care during this sensitive time.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

• Conduct psychosocial assessments to understand patients' emotional and practical needs.
• Collaborate with interdisciplinary teams to create personalized care plans.
• Provide counseling and emotional support to patients and families.
• Advocate for patients' psychosocial needs within the care team.
• Assist with advanced care planning discussions and support families during challenging times.
• Maintain accurate and confidential documentation of interventions and progress.
• Bereavement services as needed
• Attend IDG Bi-Monthly in person

POSITION QUALIFICATIONS

Prior Hospice experience-1 year (Required) Master's degree in Social Work from an accredited institution. State licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) as required. Strong interpersonal skills and the ability to communicate effectively with patients, families, and interdisciplinary teams. Empathy, compassion, and a deep understanding of the emotional and psychological challenges of end-of-life care. Knowledge of community resources and support services for patients and families. Excellent problem-solving abilities and the capacity to navigate complex emotional situations. Ethical conduct and the ability to maintain confidentiality.
