Glendale, CA Job Details Part-time | Full-time Up to $38 an hour 12 hours ago Benefits 401(k) Qualifications Computer operation English Desktop applications Productivity software Armenian Full Job Description Adult Day Health Center (ADHC) seeking Social Worker to provide assessment and case management services to elderly and other physically/mentally impaired adults in day program setting. Responsible for assisting the team to provide all social work services at center. Will conduct assessments, quarterly reports, individual and family counseling as needed, in addition to standard case management duties (telephone calls to caregivers/hospitals, progress notes, developing/following treatment plans, attending treatment team meetings, etc •
Schedule:
Monday - Friday 8am - 2:30pm •2 locations available: Glendale, CA & North Hollywood, CA •
Qualifications:
Master's or Bachelor's degree in social work or psychology, Master's in gerontology, or Master's in counseling or AMFT. Must have excellent oral and written communication skills, with basic proficiency in Microsoft Office computer programs (additional computer program training will be provided) Must be energetic, self-motivated and efficient with completing tasks and meeting deadlines Knowledge of community resources in Los Angeles and surrounding areas a plus Bilingual (Armenian)
