The Social Worker collaborates with the multydisciplinary team to develop and implement individualized care plans, addresses psychosocial barriers to health and independence, assists with community resources and benefits, supports caregivers, monitors participants' safety, documents required documentation on a daily basis.

Requirements:

Candidate must have at least a BA in Social Work, Sociology, Psychology or Gerontology.

Pay:

$26.00 - $27.00 per hour Expected hours: 30.0 per week
