Job Description
Job Overview Conduct psychosocial assessments of participants, including emotional, social, family, financial, and environmental needs. Develop and update individualized care plans in collaboration with participants, families, caregivers. Provide individual, family, and supportive counseling to address emotional, behavioral, and social concerns. Monitor participants for changes in psychosocial status and communicate significant concerns to the interdisciplinary team. Assist participants and families with accessing community resources, including housing, transportation, food assistance, financial assistance, and healthcare services. conduct daily documentation Qualifications
• At least Bachelor's Degree in Social Work ,Sociology, Psychology or Gerontology from an accredited porting individuals through complex health challenges. We value energetic professionals committed to compassionate care and continuous learning.
Pay:
$25.00 - $26.00 per hour Expected hours: 30.0 per week
