Duties Conduct thorough assessments to identify client needs, including behavioral health, addiction issues, disabilities, and geriatric concerns Develop personalized care plans by coordinating with community resources such as Medicaid, Medicare, Section 8 housing, and workforce development programs Provide crisis intervention and crisis management to stabilize clients during emergencies or emotional distress Utilize motivational interviewing techniques to motivate clients toward positive behavioral changes and goal achievement Offer addiction counseling support and behavioral health guidance tailored to diverse populations, including seniors and individuals with disabilities Identify appropriate social services resources and facilitate access for clients in need of housing, healthcare, or financial assistance Communicate effectively with clients through written reports, case notes, and client communication strategies while maintaining confidentiality and professionalism Experience Proven experience working in social services or related fields such as social work or case management Demonstrated ability to handle crisis situations with calmness and efficiency using crisis intervention skills Knowledge of behavioral health issues, addiction counseling methods, and working with culturally diverse communities Familiarity with Medicaid, Medicare, Section 8 housing programs, and workforce development initiatives Experience working with seniors, individuals with disabilities, or geriatric patients is highly desirable Strong writing skills for documentation and reporting purposes Bilingual abilities are a plus to better serve diverse client populations; proficiency in additional languages enhances communication effectiveness CNA certification is prefered Embark on a rewarding career where your dedication transforms lives!
