Social Service Designee Meadowbrook Post Acute - 2.3 Hemet, CA Job Details Full-time From $20 an hour 1 day ago Benefits Health insurance Dental insurance Paid time off Qualifications Organizational skills Full Job Description Meadowbrook Post Acute is seeking a compassionate, organized, and dedicated Social Services Designee to join our interdisciplinary team. We are committed to providing exceptional short-term rehabilitation and long-term skilled nursing care while enhancing the quality of life for every resident. As our Social Services Designee, you will play a vital role in supporting residents and their families through the admission process, care planning, discharge planning, and ongoing psychosocial support. Responsibilities Complete psychosocial assessments for residents upon admission and as needed. Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team. Assist residents and families with adjustment to the facility and provide emotional support. Coordinate discharge planning to ensure safe and successful transitions. Facilitate referrals to community resources and outside agencies. Advocate for residents' rights, dignity, and quality of life. Assist residents and families with advance directives and other social service needs. Participate in care conferences and Quality Assurance/Performance Improvement (QAPI) activities. Maintain accurate, timely, and compliant documentation in accordance with state and federal regulations. Collaborate with nursing, therapy, dietary, admissions, and other departments to meet resident needs. Support residents with concerns related to financial resources, transportation, housing, insurance, and community services when appropriate. Qualifications Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or a related field preferred. Social Services experience in a skilled nursing facility or healthcare setting preferred. Knowledge of Medicare, Medi-Cal/Medicaid, discharge planning, and long-term care regulations is preferred. Strong communication, organization, and interpersonal skills. Ability to work independently while collaborating effectively with an interdisciplinary team. Compassionate, professional, and resident-centered approach to care. Proficiency with electronic medical records and Microsoft Office applications. Why Join Meadowbrook Post Acute?
