In this vital role, you will provide comprehensive services to foster children, biological family members, and Resource Families. Your expertise will support foster children and their families through complex emotional, social, and practical challenges, while in the Foster Care System. This position offers an exciting opportunity to make a meaningful impact by coordinating community resources, conducting assessments, and delivering person-centered care tailored to each individual's needs. The ideal candidate will be energized by helping others navigate difficult life circumstances and committed to fostering positive outcomes through collaborative interprofessional efforts. Duties Conduct thorough client assessments to identify social, emotional, and environmental needs, including intake evaluations for new clients. Develop personalized planning strategies that facilitate smooth transitions from being removed from biological family members to Resource Families. Provide individual face to face motivational interviewing, and behavioral therapy techniques. Coordinate with healthcare providers, community organizations, and support networks to connect clients with essential resources. Document all interactions accurately within the guidelines of the Interim Licensing Standards. Offer crisis intervention services during urgent situations involving mental health crises or behavioral health emergencies. Engage in initiatives related to social work practice improvements or specific populations. Qualifications Master's degree in social work (MSW), Sociology and Psychology. Proven experience working with diverse populations including children, adults, and individuals with minor disabilities or developmental disorders. Strong knowledge of community resource coordination, discharge planning processes, and crisis management protocols. Excellent communication skills for effective client interaction and interprofessional collaboration within healthcare teams. Demonstrated commitment to ethical practice aligned with social work standards and cultural competency in working with individuals from varied backgrounds.
