Job Opportunity. Monday through Friday 6 hours mornings. Montebello ADHC is interested in one (1) Social Worker Assistant candidate to provide services to our Spanish-speaking Participants. Armenian-speaking also preferred but not required.Social Worker Requirements and Qualifications:
Bachelor's degree in social work, psychology, sociology, or the arts.Social Worker Job Summary:
We are looking for empathetic and supportive social worker with a genuine desire to help and improve the lives of others. Montebello ADHC is a Medi-cal funded center, serving adults and older adults of multicultural backgrounds and diversity. As a social worker assistant, you will help participants cope with difficult life stressors through providing individualized care and counseling with linkage to community resources, facilitation, and support. Social Worker Duties and ResponsibilitiesConduct Bio-psychosocial Assessments/6 months-Reassessments to assess participant's needs/goals and develop strategies to help improve quality of life, monthly flow sheets, facilitate access to community resources, elder abuse reporting and filing APS report, maintain regular communication with supervisor, collaborate with other multidisciplinary team members as needed. Evaluate supports and services to determine eligibility Provide support and resources for participants and/or caregivers Research and refer participants to referrals in community Active, empathic listening when providing supportive counseling to participants Case management, Care Coordination on behalf of participant/caregiver Advocate and raise awareness on behalf of participants Participate in monthly and quarterly in-service and trainings Proficient computer skills, including Microsoft Office (Word) Motivated to take on additional community involvement projects and solve problems Comfort in a fast-paced environment with multiple cases Self-directed and ability to work well with othersPay Scale:
