- Full-time Joy Adult Day Health Care Center, located in Norwalk, is hiring a Korean Speaking Social Work Assistant.
Overview We are seeking a compassionate and motivated Social Work Assistant to join our Adult Day Health Care (ADHC/CBAS) team. The Social Work Assistant supports participants and families through assessments, care coordination, counseling support, documentation, and resource referrals while working closely with the Social Worker and multidisciplinary team (MDT). Qualifications Legal to work in the U.S. Bachelor's degree in Social Work, Psychology, Gerontology, Human Services, Counseling, Sociology, or a related field preferred Experience working with older adults, individuals with disabilities, healthcare, social services, ADHC, CBAS, or community-based programs preferred but not required Strong communication, organizational, and documentation skills Ability to work collaboratively in a multidisciplinary team Bilingual in English and Korean Required Knowledge of Microsoft Office, Word, and Excel Ability to maintain confidentiality and professional boundaries Work Schedule Monday to Friday, 8:00 AM to 3:30 PM Responsibilities Assist the Social Worker in conducting participant interviews, assessments, reassessments, and home visits. Gather and document information regarding participants' social, emotional, family, and environmental needs. Complete quarterly reports, progress notes, and other required documentation in a timely manner. Provide supportive counseling and emotional support to participants under the supervision of the Social Worker. Assist participants and families with accessing community resources, benefits, and support services. Monitor participants' psychosocial well-being and report significant changes or concerns to the Social Worker and multidisciplinary team (MDT). Participate in multidisciplinary team (MDT) meetings, case conferences, and care planning discussions. Facilitate or assist with support groups, discussion groups, and other psychosocial activities. Maintain accurate participant records in compliance with CBAS, Title 22, Medi-Cal, and center policies. Assist with participant enrollment, orientation, follow-up services, and discharge planning activities. Perform other duties as assigned by the Social Worker, Program Director, or Administrator.
