A Family, School, or General Social Worker provides specialized assistance to vulnerable families and children.   • $30 an hour 19 hours ago Qualifications Master of Social Work Social work Filing Master's degree Bachelor's degree Psychology Social Work Full Job Description Community-Based Adult Services/Adult Day Health Care facility looking for an MSW.
Experience in ADHC is a PLUS.
Masters Social Worker:
  • Masters in Social Work; OR
  • Masters in psychology, gerontology or counseling with ONE year of experience providing social services; OR,
  • Bachelor's degree in social work with TWO years experience providing social services.
MSW Job Duties:
  • Provide assessment and reassessment
  • Development and implementation of Individual Plan of Care
  • Individual and group counseling to elderly participants as per each individual plan of care
  • Planning and coordination of discharge plan
  • Documentation of provided services
  • Provide various care management services to participants in order to obtain needed services such as IHSS and Access
  • Assist with Primary Care Physician appointments
  • Provide case management to participants in need of housing referrals and access to social welfare programs
  • Complete all relevant paperwork and maintain participants' files
