Overview We are seeking a compassionate and motivated Social Worker to join our Adult Day Health Care (ADHC) team. The Social Worker plays a key role in supporting participants and their families by providing psychosocial assessments, care coordination, and ongoing support services.
Qualifications:
Legal to work in the U.S. Candidates should hold an MSW or MA(in psychology, gerontology, or counseling and have at least one year of experience), or possess a BSW with a minimum of two years of experience in a related field Experience in ADHC, CBAS, healthcare, or geriatric settings preferred but not mandatory Strong communication, organizational, and documentation skills Ability to work collaboratively in a multidisciplinary team Bilingual in English and Korean Required Knowledge of Microsoft Office, Word & Excel Work schedule: Monday to Friday, 8:00am to 3:30pm Responsibilities Conduct comprehensive psychosocial assessments upon enrollment and as required by CBAS regulations Participate in the development, review, and update of the Individual Plan of Care (IPC) as part of the interdisciplinary team Conduct assessments and quarterly reports Provide counseling, advocacy, and education to participants and families Coordinate and refer participants to appropriate community resources and benefits Participate in interdisciplinary team meetings, case conferences, and IPC reviews Ensure documentation is accurate, timely, and compliant with CBAS, Title 22, and Medi-Cal requirements The job description is in no way meant to describe exactly what the job duties are. Pay:
$25-30 per hour DOE Benefit(After Probation Period/3 months): 40 Hours Sick Day Paid, Insurance Reimbursement $150,
