Job Description

Job Overview The Case Manager provides comprehensive case management, intervention, care coordination, and family support services to students and families served through Pleasant View Elementary and Sunnyside Elementary and surrounding communities. The Case Manager works collaboratively with families, school personnel, healthcare providers, community agencies, and other service providers to identify needs, develop and implement individualized service plans, and connect families with appropriate resources. The position focuses on supporting the overall well-being and stability of children and families by helping ensure access to medical, dental, vision, behavioral health, developmental, educational, and other supportive services. The Case Manager will maintain regular communication with families, conduct home visits, provide parent education, assist with workshops and community events, and advocate for families while maintaining appropriate professional boundaries and confidentiality. The Case Manager must demonstrate strong communication, organization, resource coordination, and relationship-building skills and be comfortable working independently while also functioning as part of a multidisciplinary team. Essential Duties and Responsibilities Conduct assessments and gather information to identify the medical, behavioral health, developmental, educational, and social needs of children and families. Develop, implement, monitor, and update individualized service plans based on identified family needs and program requirements. Coordinate services and resources with schools, healthcare providers, behavioral health professionals, community organizations, and other agencies. Work collaboratively with school officials and staff to support the needs and well-being of students and families. Assist families in accessing appropriate medical, dental, vision, behavioral health, developmental, educational, housing, and other community-based services. Provide information, education, and support to parents and caregivers regarding available services, resources, and child and family needs. Make regular home visits and maintain appropriate contact with assigned families. Provide intervention and support to assist families in developing, maintaining, increasing, and maximizing independent functioning. Facilitate referrals and follow-up to ensure families are connected with appropriate services and that identified service plan objectives are addressed. Maintain accurate and timely case records, reports, documentation, and required program paperwork. Secure and maintain required program consents and authorizations. Maintain confidentiality and protect client information in accordance with applicable laws, regulations, and organizational policies. Complete mandated reports to Child Welfare Services, Adult Protective Services, or other appropriate authorities when required. Maintain appropriate professional boundaries when working with clients, families, school personnel, and community partners. Respond to phone calls, emails, and other communications in a timely and professional manner. Maintain an accurate and current calendar and check email regularly. Participate in intake and enrollment processes for students and families requiring case management or supportive services. Participate in case consultation, team meetings, and coordination activities as required. Conduct or assist with parent education, workshops, presentations, and community events. Participate in required trainings and professional development activities. Maintain positive, respectful, and culturally responsive relationships with children, families, school personnel, and community partners. Perform other duties as assigned. QualificationsEducation and Experience Bachelor's degree in Social Work, Human Services, Psychology, Behavioral Health, or a related field preferred; or Two years of relevant experience in case management, social services, family support, behavioral health, community services, or a related field may be considered in lieu of a degree. Prior experience providing case management or supportive services to children and families is preferred. Experience working in a school, community-based, healthcare, behavioral health, or social services setting is preferred. Experience working with diverse populations and families experiencing multiple or complex needs is preferred. Experience with crisis intervention, crisis management, family support, or resource coordination is beneficial. Knowledge of community resources and social service systems is strongly preferred. Familiarity with Child Welfare Services, Adult Protective Services, housing assistance, healthcare resources, behavioral health services, workforce development, and other community-based programs is beneficial. Spanish required Skills and Abilities Strong written and verbal communication skills. Ability to establish positive and professional relationships with children, families, school personnel, and community partners. Ability to assess needs, prioritize responsibilities, and manage multiple cases effectively. Strong organizational and time-management skills. Ability to maintain accurate and timely documentation. General computer proficiency and ability to learn and utilize electronic case management or recordkeeping systems. Ability to maintain confidentiality and exercise sound professional judgment. Ability to work independently while also collaborating effectively as part of a multidisciplinary team. Knowledge of and ability to utilize community resources to support children and families. Ability to maintain professional boundaries. Ability to communicate effectively with individuals from diverse cultural, socioeconomic, and linguistic backgrounds. Ability to respond appropriately to crisis situations and identify when additional intervention or referral is needed. Compassionate, respectful, and family-centered approach to service delivery.
Pay:
$20.00 - $23.00 per hour Expected hours: 37.5 per week
