Job Summary Monarch Hospice and Palliative Care are seeking a compassionate and dedicated Part Time Hospice MSW (Master of Social Work) to join our team. The ideal candidate will provide essential support to patients and their families during challenging times, ensuring that emotional, social, and spiritual needs are met. This role requires a strong background in clinical counseling and case management, particularly within the hospice or geriatric care settings. Responsibilities Conduct comprehensive patient assessments to determine psychosocial needs and develop appropriate care plans. Provide clinical counseling to patients and families, addressing emotional and mental health concerns. Collaborate with interdisciplinary teams to coordinate care and ensure holistic support for patients. Offer guidance on resources available for patients and families, including community services. Document all interactions and interventions in compliance with medical terminology standards. Facilitate support groups or individual sessions focused on coping strategies for grief and loss. Maintain confidentiality and adhere to ethical standards in all aspects of social work practice. Qualifications Master's degree in Social Work (MSW) from an accredited institution. Valid state licensure as a social worker is preferred. Experience in clinical counseling, case management, or hospice care is highly desirable. Knowledge of geriatrics and mental health counseling practices is beneficial. Familiarity with applied behavior analysis techniques is a plus. Strong communication skills, both verbal and written, with the ability to empathize with patients and families. Proficient understanding of medical terminology related to hospice care.
