A Family, School, or General Case Manager coordinates the delivery of a range of social and health services to individuals and families. Assess client needs and work with agencies and institutions ensure clients get the help they need. Monitor client and work with other service providers as necessary to address client needs. Provide limited counseling to client and their family.

Overview:
Works directly with families to provide social and emotional support and serve as a liaison between the Family Team and the service systems serving the participants, through a mentoring/coaching relationship. Qualified candidates must have related life experience as a parent or legal guardian of a child and experience as a recipient of services. Spanish speaking preferred.

Responsibilities:
Provide "one-on-one" support for the parents of participant(s) in the Wraparound Program. Attend all scheduled Family Team Meetings. Assist Family Team members and/or Wraparound Program participant(s) with the completion of necessary paperwork or forms. Assist in securing and presenting community resources to the Family Team. Be available to provide telephone support and crisis de-escalation to youth and family, 24 hours a day/7 days a week, including holidays, through an on-call system. Meet with Parents/Caregivers on a regularly scheduled basis to support and empower caregiver.
Qualifications:
Must have a High School Diploma or equivalent. 21 years or older due to County contract requirements. Maintain a valid California Driver's License and acceptable driving record of at least 3 years. Maintain criminal record clearance Must have a philosophy of support for at-risk children and families in line with the mission, vision, values, and statement of faith of Olive Crest.
