Job Summary:
The EmPATH Social Worker is a member of a multi-disciplinary team responsible for providing social work-related services to the adult population in the crisis stabilization unit. The Social Worker coordinates care of the patient by interfacing regularly with various members of the patient's multi-disciplinary treatment team—Emergency Department providers, psychiatrist, program manager, clinical RNs and staff. The Social Worker provides assessment and case management services, including biopsychosocial and behavioral health assessments, treatment/discharge planning, group, individual and family therapy. They provide assistance to both patients and families to meet related social and emotional needs for psychiatric condition, treatment and recovery. What You'll Bring to the
Team:
Strong verbal and written communication skills Strong organizational skills Problem solving skills Patient-focused Ability to navigate and connect successfully with outside provider agencies (Health Plans, DHHS).
Licensure and Certification:
Current BLS provider card Current Associate Clinical Social Worker (ACSW) through BBS Current CA LCSW preferred Lanterman-Petris-Short (LPS) Certificate required within 60 days of employment.
Education:
Master's Degree in
Social Work Experience:
One year of hospital and/or mental health social work experience required. Care management experience preferred. Preferred 2-3 years in Behavioral Health related field Back Share Apply Now
