A Family, School, or General Case Manager coordinates the delivery of a range of social and health services to individuals and families. Assess client needs and work with agencies and institutions ensure clients get the help they need. Monitor client and work with other service providers as necessary to address client needs. Provide limited counseling to client and their family.
M/W/Th 3-7pm, T/F 2-6pm •Note that this position is split between Torrance and Gardena Make a Difference Every Day The Career Bound Case Manager plays a key role in supporting youth through workforce development, career exploration, and personal growth. This position is responsible for developing and delivering engaging after-school programming, creating a safe and welcoming environment for members, and connecting youth to career pathways, special events, and enrichment opportunities. Through case management, progress monitoring, family engagement, and individualized support, the Career Bound Case Manager helps students achieve their academic, career, and personal development goals while preparing them for post-secondary education and employment success. What You'll Do Develop and maintain a written plan for effective delivery of the Career Bound program including workshop, recruit/promotion of opportunities, and individualized case management Deliver engaging group activities and guidance curriculum that build essential life skills, self-esteem, decision-making, and problem-solving abilities Collaborate with parents, teachers, and school staff to create supportive networks, and connect students and families with community resources and educational opportunities Monitor academic performance and behavior, facilitating interventions and support when needed Implement developmentally appropriate and prevention-oriented group activities to meet member's academic needs and career goals Assists all members, individually or in groups, with developing academic, career and personal/social skills, goals, and plans. Accurately and appropriately interprets and utilizes student data Provide individual academic and career counseling through case management. Maintain comprehensive records and track student progress and program outcomes through data collection, evaluation, and reporting—for student success tracking, and grant requirements Communicate program goals and progress to members, parents, and school officials. Assists with placement and follow-up services for students including certifications, internships, apprenticeships, employment, etc. Foster a positive, inclusive, and safe learning environment using positive youth development strategies Serve as a positive role model and collaborate with staff and leadership to create and track measurable goals for student success What You Bring Strong interest in education and student success Previous experience in educational or youth development settings preferred Experience working with diverse communities and populations Excellent planning, organizational, communication, and presentation skills Strong interpersonal skills with a collaborative mindset, flexibility, and adaptability Ability to work independently in dynamic environments Ability to motivate youth and manage behavior problems Ability to de-escalate problems when present, as well as problem-solve during day-today-club activities Ability to plan and implement quality learning programs for youth Proficiency in Microsoft Excel, Word, and Office 365 Flexible schedule availability, including some Saturdays and occasional evenings/weekends Bilingual (English/Spanish) preferred 21 + years old CPR/First Aid certification (provided upon hire) Must complete all pre-employment clearances (see below) Pre-Employment Requirements All candidates must complete: Background check (including criminal history) TB test Drug & alcohol screening I-9 verification with original documents required What We Offer Employee assistance program Employee discounts Mission-driven work with a long-term impact Working Conditions & Physical Requirements Onsite at designated work location (standalone Clubhouse or school site) involving indoor and outdoor activities Ability to push/pull, squat, twist, turn, bend, stoop, lift a minimum of 25 lbs.
