AL01 Job Summary The MSW provides professional social work services to patients, clients, and their families. The role focuses on assessing psychosocial needs, providing counseling and support, coordinating resources, and assisting with care planning and discharge planning.
Responsibilities Duties:
Conduct psychosocial assessments and identify patient/client needs. Provide individual, family, and supportive counseling. Develop and implement appropriate care and treatment plans. Coordinate referrals to community resources and support services. Assist with discharge planning and transitions of care. Advocate for patients/clients and help address social, emotional, and financial concerns. Collaborate with physicians, nurses, case managers, and other healthcare professionals. Maintain accurate and timely documentation in accordance with facility policies. Provide crisis intervention and support when needed. Educate patients and families about available resources and services. Maintain confidentiality and comply with applicable laws, regulations, and organizational policies.
