We are seeking a Wraparound Facilitator to serve families throughout the Bay Area within a 2-hour radius. This rewarding position plays a vital role in helping children and families achieve stability, permanency, and long-term success through a strengths-based, family-centered approach. About the Role The Wraparound Facilitator coordinates, leads, and monitors the development and implementation of each family's Plan of Care. This role ensures that services and supports reflect the family's voice, culture, strengths, needs, and goals. Facilitators work collaboratively with youth, families, caregivers, support staff, service providers, and community partners to build a team-based plan that promotes safety, stability, permanency, and positive outcomes for children and families. Key Responsibilities Facilitate the Wraparound process using a family-driven, strengths-based approach. Coordinate and lead Child and Family Team meetings. Develop, implement, and monitor individualized Plans of Care. Partner with families, youth, caregivers, service providers, and community resources to ensure coordinated support. Advocate for family voice, choice, and cultural responsiveness throughout service delivery. Monitor progress toward identified goals and adjust plans as needed. Maintain timely and accurate documentation in accordance with agency and program requirements. Qualifications Master's degree in Social Work or a related field preferred. Experience working with children, youth, and families required. Experience in foster care, adoption, behavioral health, child welfare, or Wraparound services strongly preferred. Strong facilitation, communication, collaboration, and organizational skills. Commitment to culturally responsive, trauma-informed, and family-centered practices.
