Manages resources and patient flow by matching clinical competencies with individualized patient and unit needs. Documents ongoing status, interventions, patient responses, and outcomes in an accurate and timely manner. Uses standards of care, policies, procedures, and regulatory guidelines to guide practice. Applies quality measures to improve performance and accountability for patient outcomes and experiences. Participates in unit-based quality improvement, evidence-based practice, and research activities. Demonstrates working knowledge of the involuntary commitment process. Assesses patients' physical, psychosocial, cultural, and spiritual needs through observation, interview, and review of medical records and clinical data. Evaluates patient responses and intervenes to ensure optimal outcomes. Develops and implements patient care plans in collaboration with the interdisciplinary team. Demonstrates competency in various therapeutic and diagnostic interventions. Promotes, advocates, and collaborates to protect the health, safety, and rights of patients. - Knowledge of services available in the local community, particularly services available to patients with limited or non-existent payment resources [Required]
- Discharge Planning, Utilization Review, and Interdisciplinary Team Coordination skills [Required]
- Grief counseling and crisis intervention skills [Required]
- Knowledge of state and federal guidelines pertinent to care management [Required]
- Demonstrates the ability to maintain strict confidentiality of patient and/or customer information [Required]
- Suggest and participate in process improvement opportunities [Required]
- Ability to document patient care and complete task list in accordance with standards of care and other protocol [Required]
- Seeks solutions to resolve unexpected problems and make practical routine decisions [Required]
